Documentation Request Process

If you would like to request Court Ordered Conjoint Counseling progress reports or a final summary, please follows the steps below.

Requests can take up to 2-4 business days to process. If you are requesting a rush order an additional $30 fee applies.

Prior to picking up your documents, total payment must be paid in full.

You can submit your request for documents in-person at our main CYS lobby at
4545 N. West Ave. Fresno 93705

Monday thru Friday from 8:00am – 5:00pm.

All online requests received during non-business hours of our CYS lobby will be processed the following business day.

If you are completing the request process online, you will receive an email at the email address provided in the form above, informing you when your request has been completed and is ready for pick up. Please note you can pay for your forms in-person even if you requested them online.
